Cardinals Baseball

You may register for 2012 Cardinals Baseball one of two ways:

1. Download and print the forms below, fill them out and fax to David Hash at (801) 681-0286.
2. Open the forms, fill out online, save to your computer as .pdfs and e-mail to davidleehash@gmail.com. 

Player fees are as follows:  $225 for Fielders and $125 for Pitchers (if paid by check).  Payment should either be mailed by check or paid by credit card via one of the Pay Pal buttons at right.  There is a $10 fee added for payment by credit card.  Whichever method of payment you choose, please make sure to also submit your registration form by one of the methods listed above.
